Context :

With funding from CDC/PEPFAR Cameroon for the Atteindre95 project, EGPAF is supporting the Ministry of Public Health and the Government of Cameroon to scale-up HIV Testing Services (HTS) to ensure that 95% of people living with HIV know their status, that 95% of them are initiated on ART, and that 95% of those on ART are virally suppressed. EGPAF is also supported by other donors such as Unitaid to implement innovative approaches for Early Infant Diagnosis of HIV and catalysing Pediatric Tuberculosis services.

Job Summary :

The HIV Medical Case Manager will work closely with clients living with HIV/AIDS who have been recently diagnosed with multiple psychosocial and/or health related needs. Case management is a patient centered approach to services that includes treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining additional health and economic, legal services, social support and any other needed services. The goals of case management are to ensure that clients with HIV/AIDS have timely access to comprehensive medical care and social services, prevent disease transmission and delay of HIV progression, and to promote and support client independence and self-sufficiency. The Case Manager will be supervised by the Doctor/Nurse officer with dotted line with the facility HIV services coordinator.

Specific Duties & Responsibilities :

  • Works in collaboration with the HIV service Coordinator and clinical staff to identify eligible clients in need of case management and respond promptly whenever requests for case management services arise
  • Assess clients and work with them to develop a Comprehensive individualized Service Care. Plan which includes realistic, measurable and mutually acceptable goals that are based on the results of the assessment
  • Monitor to assess the efficacy of the plan and periodic re –evaluation and revision of the plan as necessary
  • Screen patients for psychosocial and mental health needs, and refer to appropriate services (psychosocial team, psychologist, CBOs, etc.)
  •  Propose to patients the various differentiated service delivery models available at site with their advantages and refer appropriately
  •  Provide HIV risk reduction education and prevention
  • Map existing community cardres/ organizations (community health workers, Home-based
  • care workers, Para social workers, etc.) in communities and refer patients as needed
  •  Ensure a proper and effective linkage of eligible clients to partners providing HIV support services at facility or at community level
  • Ensure that patients receive services that address the holistic needs of OVC and
  • caregivers including health, nutrition, education, protection, livelihoods, and psychosocial well-being
  •  Ensure appropriate documentation and feedback to both facility and community teams through prompt reporting and proper filling of case management forms and ensure case filing system is developed and maintained
  •  Create community linkages for broader community engagement of adolescents, youth and caregivers in case management, child protection, parenting, etc.
  •  Document progress made toward goals by writing periodic activity reports, these reports will be shared with the line manager and the HIV Services Coordinator / matron of the assigned health facility
  •  Attend coordination meetings with regards to the holistic care of the HIV
  •  Any other duty as assigned by the line manager

Minimum Requirements :

  • A bachelor’s degree in social sciences/ psychology/ Nursing with at least 2 years’ experience in HIV Management
  • Diploma in counseling/social sciences/psychology/ nursing with at least four (4) years’ experience in HIV patient management. A certificate in HIV management will be an added advantage.
  • Fluency in French is a core requirement of the job, and work knowledge of English and Pidgin is an advantage

Required competencies, skills and experience :

The counsellors must show a positive attitude towards ARV medications. Their own beliefs about the efficacy of ART and the importance of adherence is key as it would determine how they communicate adherence to the clients.

  • The counsellor must understand how HIV affects the body and the basics of ART, importance of adherence and the dangers of non-adherence, barriers to adherence and how to overcome them.
  • The counsellor should have a clear understanding of how the client’s social and economic situation and their cultural background can impact upon adherence (poverty, lack of food, shelter)
  • Need for an empathetic, non-judgmental approach towards the clients
  • Good understanding of the 5As in adherence counselling
  • Good communication, interpersonal and supervisory skills
  • Ability to work as a team and with minimal supervision
